Visualizing how magnetic susceptibility differences (ΔB0) at air-tissue interfaces interact with the low phase-encoding bandwidth of single-shot Echo Planar Imaging (EPI) to produce severe geometric distortion, intra-voxel signal dropout, and how reversed phase-encoding (TOPUP) unwarps the image.
Physical Foundations of EPI Distortion & Signal Loss:
1. Why Only the Phase-Encoding Axis Distorts: In single-shot EPI, frequency encoding along x is fast (dwell time ~ a few microseconds), giving a massive bandwidth of ~1,500 Hz/pixel — so off-resonance shifts along x are negligible (<0.05 pixels). In contrast, acquiring lines in ky requires waiting for an entire echo spacing (~0.5–1.0 ms), yielding an extremely low bandwidth along y (BWPE ≈ 20–35 Hz/pixel). Any off-resonance frequency shift Δf translates directly into a large spatial pixel displacement: Δy = Δf / BWPE.
2. Compression (A→P) vs. Stretching (P→A): Near the frontal sinuses, tissue is diamagnetic while air is paramagnetic/neutral, setting up a steep susceptibility field gradient. In Anterior-to-Posterior (A→P) phase encoding, the spatial displacement shifts the orbitofrontal cortex posteriorly, squashing it into the brain. Reversing the gradient polarity to Posterior-to-Anterior (P→A) flips the displacement sign, stretching the frontal lobe forward into the bone/air.
3. Intra-Voxel Dephasing (Dropout): Within a voxel of finite slice thickness and size, macroscopic field gradients |∇B0| cause spins across the voxel to precess at differing rates. By the time the gradient echo is sampled at echo time TE, destructive phase interference causes complete, irreversible signal blackout (dropout). Notice that while TOPUP successfully unwarps the geometry, the signal dropout in the orbitofrontal cortex cannot be recovered!
4. The "Blip-Up / Blip-Down" (TOPUP) Solution: By acquiring two brief EPI volumes with opposite phase-encode polarities (A→P and P→A), the algorithm calculates the off-resonance fieldmap Δf(x, y) that mathematically aligns both opposite distortions back to the true undistorted anatomy.
